STUDENT REPRESENTATIVE CONTRACT
Article 1 : Parties

This Health Service Agreement is between ‘’.............’’
(hereinafter referred to simply as “REPRESENTATIVE”) on one side and EDUHEALTH HEALTH AND TOURISM SERVICES TRADE LIMITED COMPANY (hereinafter referred to simply as “EDUHEALTH”) on the other side. It was signed by agreement on the following terms.
REP and EDUHEALTH together referred to as “Parties”, separately referred to as “Party”.

2.2. REP
Article 2: Definitions
EDUHEALTH Services https://eduhealth.com.tr/en
EDUHEALTH: Means International Travel and Health Agency.
Mediator REP: means ordinary people or those who are committed to the government and direct students to take advantages of International Travel and Health Agency EDUHEALTH.
Service: means the Service that the mediator REP provides by directing students to EDUHEALTH.
Service fee: means the fee which offered by EDUHEALTH to the mediator REP for the service, which in turn provided by him to EDUHEALTH.
Article 3: the Subject of the Contract
The theme of this contract includes the rights and responsibilities of both parties in relation to the work of mutual work between the company EDUHEALTH and REP.
Article 4: Rights and Responsibilities of EDUHEALTH
4-1 EDUHEALTH agrees and undertakes to make maximum effort to ensure that the students to be directed by the REP receive the best education under the best conditions.
4-2 EDUHEALTH agrees and undertakes to pay 50% of this invoice amount, excluding VAT, to the REP by submitting a copy of the service fee invoice to the REP in accordance with the agreements it has made with the relevant universities and educational institutions, despite the university promotion fee notified to it from these institutions.
4-3 EDUHEALTH offers and undertakes to give the percentage of REP according to the shape referred to in Article [4-2] within 7 (seven) days after cutting his bill without waiting for the hospital payment offered to him.
Article 5: REPRESENTATIVE Rights and Responsibilities
5.1 REP offers and undertakes to provide the mediation service under this contract in business-related activities that fall within the EDUHEALTH.
5.2 REP shall not come out the framework of the information found in the above introduced structure of EDUHEALTH.
5-3. REP shall not ask or get money outside the EDUHEALTH.
REP is not entitled to open a site or an electronic health newspaper by name or of resemblance to EDUHEALTH inside or outside the state. And that the provisions of this article for each award rights and arenas Brand names registered in the name of EDUHEALTH.
Article 6: Privacy
6.1. The Parties agree and certify that they will not share the confidential information they have acquired about each other with third persons in written or verbal form without the approval of the other Party.
6.2. However this clause cannot be implemented in situations where the implementation of the responsibilities or valid law require otherwise. The Parties have definitely stated that they will keep the personal, medical and confidential information they acquire while providing services.
Article 7: Terminate the Contract
7.1 Any of the Parties can terminate the agreement unilaterally without stating any reason under the condition that it is notified one month (30 days) in advance.
7.2 In the case of expiration of the contract or its dissolution for any reason, the rights and responsibilities of the parties generated until that date will be terminated on condition that with debts reserved without any compensation.
Article 8: Duration of the Contract
The duration of the contract lasts for 1 (one) year starting from the date of signing of the contract.
This contract will be renewed automatically if the two parties do not have a prior notice of renewal at least 15 (fifteen) days prior to the expiration of the contract.
Article 9: Other Provisions
9.1 The terms of this contract will not be changed without written agreement of both parties after the executive date of the contract. And if there is any change in writing the contract,it will be calculated as one of its additions and will benefit rule.
9.2 The two parties have to accept that the Anatolian Courts and Executive Directorates in Istanbul are the decision makers in case of any forms resulting from the application and interpretation of this contract.
9.3 The two parties have to accept that the titles listed in the contract are at the same time the titles of notifications.
9.4 It is not entitled for the two parties to pass their rights and responsibilities to the third person.
9.5 Explanation and application of this contract is related to the Turkish rights and law.
9.6 Additions on this contract are integral part of the contract and any change on the contract will be done through the writing, agreement and signature of the two parties.
Article 10: Taxes
All taxes and fees are paid by both parties in addition to those that are sealed with held for this contract.
This contract consists of 2 (two) newspaper and 10 (ten) materials; been approved and signed in the date of
